Monitoring Anti-Tnf Drug Treatment in Inflammatory Bowel Disease. Is it Useful in Clinical Practice?

چکیده

Submit Manuscript | http://medcraveonline.com Despite their efficacy, approximately 30% of patients do not respond to these therapies, and up to 40% of those who do respond lose efficacy over time [6]. To date, when faced with the failure to respond, the approach was to act empirically by intensifying the employed anti-TNF or by changing to another anti-TNF or a drug with a different therapeutic target. However, we need to find strategies that optimise these treatments, taking into account both the pharmacokinetics and pharmacodynamics [7,8].

Utility of measuring serum concentrations of anti-TNF agents and anti-drug antibodies in inflammatory bowel disease.

Tumor necrosis factor alpha (TNFα) is a cytokine with a critical role in the pathogenesis of some chronic inflammatory diseases, such as inflammatory bowel diseases. Anti-TNF agents, which neutralize the biological activity of TNFα, are widely used among the different therapeutic options for the treatment of patients with inflammatory bowel diseases. These drugs are very useful in clinical prac...

Biologic Concentration Testing in Inflammatory Bowel Disease

Anti-TNF medications have revolutionized the care of patients with inflammatory bowel disease. However, despite an initial robust effect, loss of response is common and long-term results are disappointing. Much of this lack of durability may be due to inadequate dose optimization, and recent studies suggest a correlation between serum drug concentrations and clinical outcomes. Currently, in cli...

Is the Therapeutic Drug Monitoring of Anti-TNF Agents Necessary in Korean Inflammatory Bowel Disease Patients?

Inflammatory bowel diseases (IBD), including Crohn's disease and ulcerative colitis, are idiopathic chronic intestinal inflam-matory diseases, characterized by recurrent episodes of relapse and remission of disease. The introduction of anti-tumor necro-sis factor (anti-TNF) agents has greatly improved the treatment outcomes for IBDs, enabling higher rates of clinical and endo-scopic remission i...

Therapeutic Drug Monitoring in Children and Young Adults With Inflammatory Bowel Disease: A Practical Approach.

Therapeutic drug monitoring (TDM) is the clinical practice of measuring drug concentrations or metabolites to attain a targeted concentration in a patient's bloodstream, thereby optimizing individual dosage regimens. With the well-established knowledge of the relationship of the genetic variability of thio-purine metabolism driven by the thiopurine S-methyltransferase pathway, and the recent da...

Therapeutic Drug Monitoring of TNF Antagonists in Inflammatory Bowel Disease.

Although tumor necrosis factor (TNF)-α antagonists play a critical role in the treatment of moderate-to-severe inflammatory bowel disease (IBD), several factors can impact treatment response. The degree of systemic inflammation, serum albumin concentration, disease type, body mass index, gender, concomitant therapy with immunosuppressive agents, and especially development of antidrug antibodies...
